Is there any information available concerning the lease of small cell attachments to a utility pole? (i.e. where provider/operator attached small cell to utility pole and thereafter leases that small cell to other providers).

It depends who owns the DAS system. Some are owned by the cell carriers themselves, others by the major tower companies. The rates vary dramatically and are heavily effected by what the site covers. There is obviously a difference if a DAS system covers, say the Lincoln Tunnel in NYC, than say a small residential area in Staten Island. I would need to know more specifics to give you an accurate answer.
